I. SHPDA Administrative Report

AL2015-024, Greystone Surgery Center LLC, d/b/a Greystone Surgery Center, Birmingham, AL: Proposes to establish an Ambulatory Surgical Center through the conversion of an existing surgical center originally constructed as part of a physician's private practice. The project was approved with limiting language by order of the administrative law judge. No vote is required because the recommended order of the administrative law judge becomes the final order of the agency when no exceptions have been filed.

    AL2013-059, Renal Treatment Centers-Southeast LP, d/b/a East Jefferson Dialysis: Proposes to establish an end stage renal disease (ESRD) facility consisting of two existing hemodialysis stations to be relocated from DaVita's Birmingham East Dialysis, plus add eight new hemodialysis stations.

    Project Modification 1: Change the d/b/a name from East Jefferson Dialysis to Springs Dialysis, relocate two approved but undeveloped hemodialysis stations from Magic City Dialysis, and add one home training station, which increased the total estimated project cost from $4,755,000 to $5,715,000. Project Modification 1 approved August 19, 2014.

    Project Modification Request: Request to relocate two new stations from Birmingham Gateway Dialysis and two existing stations that were to be relocated from Birmingham East Dialysis to Birmingham Gateway Dialysis, which would give Springs Dialysis a total of 17 stations and increase the total cost from $5,715,000 to $6,260,182.     Project Modification Request: Request to relocate six CON-approved but undeveloped stations from Birmingham Gateway Dialysis to Crown Dialysis; two remaining CON-approved but undeveloped stations from Gateway Dialysis; and two existing stations that were to be relocated from Birmingham East Dialysis to Gateway Dialysis to Springs Dialysis instead, along with all related AL2013-067, CON 2654-ESRD-MOD1 costs.
