A. AL2015-030, Lakeshore Foundation, Birmingham, AL: Proposes to construct a two-story addition containing patient rooms, a nursing station, a day room and a lobby area. The Hospital consists of twenty (20) private rooms and forty (40) semiprivate rooms. When the project is complete, there will be sixty-four (64) private rooms and eighteen (18) semiprivate patient rooms. Opposition: None.

AL2015-031, Bio-Medical Applications of Alabama, Inc. d/b/a BMA of Montgomery Baptist, Montgomery, AL: Proposes to relocate and expand its existing twenty-five (25) station ESRD Treatment Center by adding seven (7) in-center hemodialysis stations and one (1) isolation station for a total of thirty-three (33) stations.

RV2015-045, Renal Treatment Centers – Southeast, LP d/b/a Davita Hokes Bluff requests to convert the existing one (1) home training station into an in-center hemodialysis station for a total of ten (10) in-center hemodialysis stations. Status: Non-Reviewable

B. Pending Reviewability Determinations

RV2014-028: Surgicare of Mobile, Ltd., requests to add five (5) operating rooms, 20 pre-/post-op bays, and four (4) restrooms and expand the waiting room and business office. Status: Pending
