United States: Alabama CON Report - August 2015

I. SHPDA Administrative Report

AL2015-024, Greystone Surgery Center LLC, d/b/a Greystone Surgery Center, Birmingham, AL: Proposes to establish an Ambulatory Surgical Center through the conversion of an existing surgical center originally constructed as part of a physician's private practice. The project was approved with limiting language by order of the administrative law judge. No vote is required because the recommended order of the administrative law judge becomes the final order of the agency when no exceptions have been filed.

II. Certificate of Need Program

  1. Project Modification

    AL2013-059, Renal Treatment Centers-Southeast LP, d/b/a East Jefferson Dialysis: Proposes to establish an end stage renal disease (ESRD) facility consisting of two existing hemodialysis stations to be relocated from DaVita's Birmingham East Dialysis, plus add eight new hemodialysis stations.

    Project Modification 1: Change the d/b/a name from East Jefferson Dialysis to Springs Dialysis, relocate two approved but undeveloped hemodialysis stations from Magic City Dialysis, and add one home training station, which increased the total estimated project cost from $4,755,000 to $5,715,000. Project Modification 1 approved August 19, 2014.

    Project Modification Request: Request to relocate two new stations from Birmingham Gateway Dialysis and two existing stations that were to be relocated from Birmingham East Dialysis to Birmingham Gateway Dialysis, which would give Springs Dialysis a total of 17 stations and increase the total cost from $5,715,000 to $6,260,182. Opposition: None.

  2. Project Modification

    AL2013-067, Renal Treatment Centers-Southeast LP, d/b/a North Jefferson Dialysis: Proposes to establish a new state-of-the-art ESRD facility consisting of two existing hemodialysis stations to be relocated from Birmingham East Dialysis, plus the addition of eight new home dialysis stations.

    Project Modification 1: Change the d/b/a name to Birmingham Gateway Dialysis and increase the estimated cost for the project from $4,490,000 to $4,957,500. Project Modification 1 approved August 12, 2014.

    Project Modification Request: Request to relocate six CON-approved but undeveloped stations from Birmingham Gateway Dialysis to Crown Dialysis; two remaining CON-approved but undeveloped stations from Gateway Dialysis; and two existing stations that were to be relocated from Birmingham East Dialysis to Gateway Dialysis to Springs Dialysis instead, along with all related AL2013-067, CON 2654-ESRD-MOD1 costs. Opposition: None.

  3. AL2015-025, The Health Care Authority of Athens and Limestone County, d/b/a Athens Limestone Hospital: Proposes to relocate/expand its outpatient surgical services and certain support services from the current hospital campus to a new 90,000-square-foot off-campus multispecialty medical office building. Opposition: None.
  4. AL2015-026, Bio-Medical Applications of Alabama Inc., d/b/a Fresenius Medical Care North Mobile: Proposes to construct and operate a new ESRD facility consisting of 13 dialysis stations, one of which will be an isolation station. Opposition: None.

  5. AL201-027, Bio-Medical Applications of Alabama Inc., d/b/a Fresenius Medical Care Alexander City: Proposes to expand the existing ESRD facility consisting of 13 in-center hemodialysis stations and 1 home training station by adding 6 hemodialysis stations, 1 isolation station and 2 home training stations for a total of 23 stations. Opposition: None.

  1. Reviewability Determinations

    RV2015-018: Regional Health Management Corporation requests to lease space to operate two physicians' offices and then sublease to RMC-Anniston for operation of a hospital-based diagnostic imaging center offering MRI, CT, general radiology, and ultrasound. Status: Nonreviewable

    RV2015-020: Southern Alabama Surgery Center LLC, d/b/a Surgery Center South, requests to expand the existing surgery center by leasing 7,569 square feet of shell space adjacent to the existing ASC space, within which Surgery Center South proposes to construct six additional operating rooms, two of which will be licensed, certified, fully staffed, and operated immediately upon completion of the construction, and four of which will remain unstaffed and held for future use. The leased space will also include 16 bays for preoperative and recovery care, counseling space, a cleaning station, a waiting area, restrooms, and a small physicians lounge. Status: Nonreviewable

    RV 2015-032: Cloverdale Health Care Inc., d/b/a Cloverdale Rehabilitation and Nursing Center, requests to construct additional space for the existing therapy department and dining area and to add an exit door to the activity day room to provide access to an exterior fenced area. Status: Pending

    RV 2015-033: Renal Treatment Center-Southeast LP, d/b/a Colonel Dialysis, requests to establish a new 10-station dialysis facility consisting of nine in-center hemodialysis stations, one of which will be a hemodialysis isolation station, and one a home training station to be used alternately for home hemodialysis training and peritoneal dialysis home training in Cullman County. Status: Pending

    RV 2015-034: Baptist Medical Center-Princeton requests to renovate existing space for the addition of a second Electrophysiology Lab (EP Lab). Status: Pending

    RV 2015-035: Dialysis Services of Decatur LLC requests to establish and operate a new 10-station dialysis facility through a joint venture in which Dialysis Clinic Inc. will be the majority interest holder and manager. Status: Pending
