March 1, 2015 is the deadline for providing notification to the
Secretary of the Department of Health and Human Services regarding
breaches of unsecured protected health information that were
discovered in the 2014 calendar year and that affected fewer than
500 individuals.
Each breach requires its own notification, which must be
submitted electronically. A description of the notification
requirements and the electronic submission form can be found here.
