On August 26, the Department of Health and Human Services (HHS)
announced that
it would provide $35.7 million in funding to 147 health
centers in 44 states, the District of Columbia and Puerto
Rico for construction and renovation to support
patient-centered medical homes.
The patient-centered medical home model for delivery of care, an
initiative created under the Affordable Care Act (ACA), "is
designed to improve quality of care through team-based coordination
of care, treating the many needs of the patient at once, increasing
access to care, and empowering the patient to be a partner in their
own care," according to the HHS press release. The model
is similar, in some respects, to the Accountable Care Organization
(ACO) initiative for care coordination that was also introduced by
the ACA.
The HHS release noted that nearly 1,300 health centers operate more
than 9,200 service delivery sites that provide care to over 21.7
million patients in the U.S.
