AL2005-039, Athens-Limestone Hospital, Athens, AL: Proposes to establish a 40-bed long-term acute care hospital, hospital within a hospital, by converting 40 acute care beds.

B. Project Modification

L2012-026, Holding Realty I, LLC d/b/a Vestavia Health Realty, LLC, Vestavia Hills, AL: Proposes to relocate 120 nursing home beds from the Jefferson Rehabilitation and Health Center ("Ketona") to a rehabilitation unit, a 24-bed rehabilitation unit, a 24-bed memory care and dementia unit, and a 36-bed long-term care unit.

Project Modification Request: Increase in the project cost from $26,565,220 to $30,857,677, or an increase of approximately 16.2%, and change the name from Holding Realty I, LLC d/b/a Vestavia Health Realty, LLC, to Cahaba River Realty, LLC d/b/a Cahaba River Health & Rehabilitation, LLC. Opposition: None.

C. Project Modification

AL2012-027, Holding Realty I, LLC d/b/a Hoover Health Realty, LLC, Hoover, AL: Proposes to relocate 118 nursing home beds from the Jefferson Rehabilitation and Health Center ("Ketona") to Hoover Health Realty. Sixty beds will be used as a short-term rehabilitation unit, 24 beds as a memory care and dementia unit, and 34 beds as a traditional long-term care unit.

Project Modification Request: Increase in the project cost from $25,595,220 to $30,408,509, or an increase of approximately 18.8%, and change the name from Holding Realty I, LLC d/b/a Hoover Health Realty, LLC, to Green Valley Health Realty, LLC d/b/a Green Valley Health & Rehabilitation, LLC. Opposition: None.

D. AL2013-073, Birmingham Senior Services, LLC d/b/a Regency Birmingham Memory Care, Birmingham, AL:

Proposes to relocate 70 specialty care assisted living beds from Tannehill Specialty Care, located on Coleman Lake Road, McCalla, AL, to Regency Birmingham Memory Care, located on West Oxmoor Road. Opposition: Rittenhouse Senior Living of Hoover, LLC, filed a letter objecting to this project. Alabama Somerby, LLC, withdrew intervention and opposition December 23, 2013.

E. AL2014-001, Bio-Medical Applications of Alabama, Inc., Montgomery, AL:

Proposes to expand the existing ESRD facility consisting of 20 stations by adding one peritoneal dialysis station. Opposition: None

Proposes to convert 16 assisted living beds to 16 specialty care assisted living beds, pursuant to Alabama Administrative Code r. 410-2-4-.04 of the State Health Plan. Opposition: None

I. AL2014-005, MH RDM Operating, LLC, Madison, AL:

Proposes to construct a new 32-bed specialty care assisted living facility by relocating 16 SCALF beds from Agape I and 16 SCALF beds from Agape II. Opposition: None
